The iliotibial band is a thick strip of connective tissue that runs along the outside of the thigh from the hip to just below the knee. It isn't a muscle and it doesn't stretch the way muscles do. When it becomes irritated, usually from repetitive knee flexion and extension during running, cycling, or jumping, it creates friction against the lateral femoral condyle, the bony prominence on the outside of the knee. That friction causes the sharp, burning pain most people feel at the outer knee.
Foam rolling and stretching target the IT band itself, but the actual problem is usually at the tissue where the band interacts with the knee or hip, not along the length of the band. When that tissue becomes chronically irritated and the surrounding structures tighten in response, the condition often plateaus. Patients can manage it enough to train through it, but it returns with every increase in volume or intensity.
Radial shockwave therapy is applied to the lateral knee tissue and the iliotibial band attachment points where friction and irritation have accumulated. The acoustic waves stimulate blood flow and the repair response in the chronically stressed tissue. Most courses run 3 to 6 sessions spaced about one week apart. Individual results vary.
